GALES FERRY LOCATION ONLY:  ONLY Urgent care is available after 7:00pm.  We provide urgent care for our patients on Saturday from 9:00am to 12:00pm at the Gales Ferry office.An AFTER HOURS FEE will be billed to your insurance carrier for appointments scheduled after 9pm M-F and Saturdays after 12pm.  The office is closed on Sundays and most national holidays, but there is always a doctor on call for our patients to assist you in an emergency (weekend/holiday fees apply)  PHYSICALS AND MED CHECKS ARE NOT CONSIDERED URGENT AND ARE NOT BOOKED DURING THESE HOURS AND ON WEEKENDS.  These hours are for urgent sick care only.  We do not book new patient sick appointments after 5pm and on the weekends.
 

NEW PATIENTS
    Plan to arrive at least 15 minutes prior to your first appointment. Be sure to bring your child's health insurance card, a valid photo I.D., and immunization record. If you are transferring care from another practice, please contact your former physician well before your appointment to have your child's records transferred. 

APPOINTMENTS

        All children are seen by appointment only. We do not offer walk-in visits. We realize children sometimes need immediate attention, but we ask you to call our office first, because we may feel your child needs to go directly to the emergency room.  To ensure that our patients are seen in a timely manner, please arrive 15 minutes prior to your scheduled appointment. This allows time to complete necessary paperwork and update insurance information. Please present any forms to the front desk at the beginning of your visit.  At each visit, the front desk receptionist will ask you to verify your child's date of birth and demographics on file.  The ensures that the doctors and staff have accurate informaiton on file at all times.

Out of Town visitors:  We do not offer any walk in visits or sick child visits on an as needed basis to non-established patients.  This includes but not limited to : weight checks, emergent visits, sick visits, well visits.  Due to the continuity of care issues, we ask that you call your regular pediatrician for advice on the closest ER or walk in clinic to be seen.  We apologize for any inconvenience that this may cause you, but as a pediatrician's office, we like to ensure that the child get's the best care possible and is referred to their own pediatrician for help.  We do not establish new care for patients who are in the area visiting on a temporary basis.
 

    If you prefer to see a particular doctor, tell our staff when making your appointment. For non-urgent and well-child visits we recommend scheduling an appointment well in advance. Urgent care and sick visits depend upon the daily schedule and which doctors are available to see your child.
 

    Our triage staff books appointments based on information given at the time of your call. Therefore, it is important to let us know if you have specific concerns about your child, so we can allot adequate time for your appointment. If you are being seen for a brief sick visit and have questions about unrelated, non-urgent issues, the doctor may ask you to schedule a separate appointment to discuss those concerns in detail.
 

    We will try to coordinate appointments so that siblings can be seen at the same time, however, we do not book back to back physical/well child appointments if you have missed one prior.   If, after scheduling an appointment for one child, you feel a sibling needs to be seen as well, please call ahead to make arrangements.
 

    We strive to keep office wait times to a minimum, but sometimes an unforeseen problem can cause your physician to run behind schedule. We will try to notify you of this when you arrive. If you cannot wait to be seen, we will assist you in rescheduling your appointment.

Please Carefully Read The Following Office Protocols:

24 hour notice -CANCELLATION / NO SHOW POLICY
 We require 24 hours notice for cancellations. Failure to give notice of cancellation will be noted as a "no-show": there is a $50.00 fee for “no-show” appointments. Although we understand how easy it can be to forget an occasional appointment, we reserve the right to dismiss families who habitually miss appointments.    You can always call the office during any hours and leave a mesage with our answering service.                                                                                  

 

LATE Arrivals:

The issue of "late patients" is a challenging one. We realize that our patients may face many challenges getting to Gales Ferry Pediatrics (such as transportation), therefore, If you have transportation issues, please be sure to express this to the receptionist at the time your appointment is being made. Our Patients are expected to arrive to their appointment 15 minutes prior to their appointment and this will be made aware to our patients when the appointment is made and at the reminder phone call. Therefore, a patient is considered "LATE" when they are 10 minutes past the appointment time and 25 minutes past the time they are supposed to arrive. Showing up 10 minutes past the scheduled time will result in being no showed for your appointment and it will need to be rescheduled.

Patients arriving 10 minutes late for a scheduled well child physical and/or a med check visit will be asked to reschedule their appointments.  Patients arriving 10-15 minutes late for a scheduled SICK visit may need to reschedule their appointment at the provider's discretion subject to the $50.00 fee.

HOSPITALIZATION
    We admit children in need of hospitalization to Lawrence and Memorial Hospital in New London, Connecticut. If a child has a problem  or condition that requires management by a specialist or in a more intensive care setting, we use the Yale-New Haven Hospital, , the Connecticut Children's Medical Center in Hartford, or Hasbro Children's Hospital in Providence.

FORMS / MEDICATION REFILL
    We complete a physical form at well-child appointments and we ask you to retain a copy of this for use during the year. If you lose your form and require an additional copy, we are happy to provide one for you for a $10.00 per copy fee.   Filling out forms are very time consuming by the staff and providers, therefore, we require a $10.00 fee per form for 24 hour completion requests filled out, this includes, but not limited to Camp/Daycare/School (except Medications forms) and Sports forms.   Because staffing is limited after hours and on weekends, please call Monday - Friday before 5:00 pm with all requests for forms and medication refills. Please allow 7 days for refills and form completion. The parent sections of forms should be filled out completely including all medicines your child is taking, along with the dosage of each medicine, at the time the forms are dropped off. This will facilitate their completion as quickly as possible. 

Violence/Abuse Protocol:  Our office does not tolerate violence/abuse in the workplace.  Any parent/patient who causes any violence or abuses staff, doctors, and/or other parents/patients in the practice will be dismissed.  This includes, but not limited to: Vulgar language, physical abuse, threats, harrassment of any type.

 

CONTROLLED SUBSTANCE POLICY:  A VALID PHOTO ID IS REQUIRED FOR CONTROLLED SUBSTANCE PRESCRIPTIONS BEING PICKED UP IN THE OFFICE.  THESE CANNOT BE MAILED.  If a person comes in that is not on the account, we will not be able to give the RX to them without a valid consent.  No Exceptions are made.  If  there is an error on the written RX, we ask that you return it to the practice for the provider.

INSURANCE /BILLING FOR SERVICES

Parents are expected to pay amounts due for office visits or copayments at the time of each visit unless other arrangements have been made in advance.  Statements of account are mailed monthly by Northeast Medical Group and are payable upon receipt. Credit card payments made in person or by telephone are acceptable. charged for service, please contact Alternate payment arrangements are available for our patients when personal or financial problems arise. At times of need, we will discuss them with you.Although we accept most insurance plans, please call ahead to confirm your coverage. a. If we do not participate in your plan, or if you are uninsured, full payment for services is required at each appointment.Balances that are in collections or are over $100.00 will not be granted a well child visit until the account is cleared up with the YMG billing department.

RECORDS REQUEST/TRANSFERIf you request copies of medical records to be sent to another physician, there will be a charge of $0.65/page, plus the fee for mailing.  Please provide us at least 30 days notice.

 

***** Attention parents, step-parents, & Guardians:   If your child is coming into the office with another person, such as aunt, grandparent, etc, please inform the practice of this.  Due to HIPAA policies, a written note from a legal "authorized" parent/stepparent/guardian is REQUIRED for all children if they need to receive any type of procedure or treatment and a valid phone number of the parent.  If a form is being picked up or records or prescriptions, please inform the office prior to the pick up so that they are aware of who is coming into the office to obtain this.  If the practice is not aware of the person and has not been added to the account as a contact or informed prior, we will not be able to distribute any forms and/or prescriptions.  A PHOTO ID is required for any other person picking up paperwork or bringing a child in that is not the parent/guardian.  Thank you for your cooperation in this matter.
